2019 Benefits Information

Few things are as important as managing your health and having access to high-quality health care.  Gates offers three optional medical plans through UnitedHealthcare's Choice Plus Network.  The CDHP 1, CDHP 2, and PPO plan cover the same services and provide comprehensive coverage to protect your health.

NOTE:

Employees at the following work locations will participate in the UHC Preferred Provider Network (PPN):  Denver, CSC, ETown, Siloam Springs, Rochester Hills, Chambersburg, Galesburg, Lithonia and Hebron.  Please refer to the Summary Benefit Coverage (SBC) documents notated with PPN if you work at one of these locations.

UnitedHealthcare - Medical Coverage

Gates offers three medical plan options in 2019 - the CDHP 1, CDHP 2, and PPO - through UnitedHealthcare.

CVS/caremark - Prescription Coverage

If you enroll in a Gates medical plan, you will automatically receive your prescription drug benefits through CVS/caremark. The pharmacy program includes a number of utilization programs to control quality, safety, and the cost of prescription drugs.

MetLife - Critical Illness

MetLife Critical Illness Insurance pays you a lump-sum benefit upon diagnosis of any one of the covered critical illnesses.  This coverage can complement existing medical coverage and help fill financial gaps caused by out-of-pocket expenses such as mortgage payments, college tuition, hiring household help, or treatment not covered by your medical plan.  Benefits are paid regardless of what is covered by medical insurance.  Payments are made directly to covered employee to spend as they choose.

MetLife - Accident Insurance

Accident Insurance provides coverage for accidental injuries that occur off the job helping to fill financial gaps caused by out-of-pocket expenses such as deductibles, co-payments, and non-covered medical services.  Benefits are paid regardless of what is covered by medical insurance.  Payments are made directly to you and you choose how to use the money.

MetLife - Dental Coverage

Dental health is an important part of your overall health. Gates offers a base dental plan (Dental Plan 1) and a second buy-up plan (Dental Plan 2) with a higher annual maximum, administered by Metlife, so you can get the dental care you and your family need.  The Dental PPD Plans provide access to a large network of dentists.  As with the medical plans, you can save money by using network providers.

VSP - Vision Coverage

Regular eye exams are an important part of overall health. Gates provides optional comprehensive vision coverage through Vision Service Plan (VSP). VSP provides a benefit for exams and materials on a yearly basis. You have the freedom to choose any vision provider, but the plan generally pays better benefits when you receive care from VSP network providers.

Securian - Life and Accidental Death Insurance

To help protect you and your family from the financial challenges that can result from death or serious injury, Gates provides all eligible employees with Basic Life Insurance and Basic Accidental Death and Dismemberment (ADD) insurance.  This coverage is provided at no cost to you.  You can also purchase supplemental life and ADD insurance for yourself and your dependents.  

Aetna - Disability Coverage

A disabling injury or illness can happen to anyone, and can last days, weeks or months. To provide important financial protection, Gates offers all eligible employees short-term and long-term disability coverage at no cost to you.  In addition, you may choose to purchase additional LTD coverage to increase your benefit.  During benefits open enrollment, you elect optional buy-up LTD which increases your LTD benefit amount.

Hyatt Legal

You may choose to purchase legal advice and services through Hyatt Legal Plans. The plan offers access to a national network of over 10,000 professional attorneys.  The Plan provides coverage for attorney fees for a wide variety of legal services.   For more information visit www.info.legalplans.com and enter the Gates code 4360010.

Magellan - Personal Resilience Program (formerly known as EAP)

Gates provides a Personal Resilience Program at no cost to you.  This service provides personal consultation services to assist a member to resolve a range of personal problems, including, but not limited to:

  • Work-related issues;
  • Parenting concerns;
  • Marriage and family distress;
  • Relationship issues;
  • Use and misuse of alcohol and drugs, and co-dependency issues;
  • Stress related to financial and legal problems;
  • Emotional stress;
  • Life crisis; and
  • Similar concerns.

In any calendar year, a member can participate in up to five (5) sessions (of 50 minutes each) for each specific problem or set of problems. If the counselor determines after one or two sessions that a member's problem or condition cannot be resolved with brief counseling, the counselor will assist the member to transition out of the employee assistance program to a provider who is appropriate for his or her condition. At that point, no further sessions are available to that member, except in connection with the transition to another provider.
